in Build Build Tools Simply click the Restore link and select the question or page you wish to restore from the dropdown menu. If you select a page, all questions that were present when it was deleted will be restored!

To access table view click the table icon in the upper-left corner of your survey list. Click the Columns link in the upper-right corner survey list. Select the checkbox for Created by and click Save.
