A Form project is identical to a Survey.
- To create a form, navigate to the SurveyGizmo home page.
- Click the arrow next to the Create a Survey button to see the additional types of projects.
- Select Create Form.
- Give your form a name and click Start Building.
Now you're ready to get started adding questions! We recommend using a Contact Form question type if you are looking to collect contact information from your form respondents.

@Tina: Thank you so much for reaching out to SurveyGizmo Support!
I suggest adding the contact Form question type to your survey. Follow the documentation below for more information on implementing this in your survey:
https://help.surveygizmo.com/help/contact-form
I hope this helps clarify!

How do I add a section to fill in name, address and email? Thank you.

@Keith: Thank you for your question!
I'm sorry for the trouble! At this point, there is not a difference between a survey and a form. Previously, forms automatically included the 'Contact Form' question type but that is no longer the case.
I will note this with our Product and Engineering teams so that this can be addressed in the application.
Thank you again for this note.

@Andi: Thank you for your question!
Based on your question, I would say that the CSV/Excel method should do the trick:
https://help.surveygizmo.com/help/excel-export
This method will allow you to export a spreadsheet of your form data where each row of the spreadsheet contains a single respondent's answers.
I hope this helps!

Hi! I'm wondering what the best option is for exporting form responses? I don't want these aggregated since I need to see individual responses. I have used the Bulk PDF export option, but is there another option?
